Adult Day Services:

  AD Centers provide comprehensive programming specially tailored for older persons and persons with disabilities who need supervision and assistance during the day. They are non-residential, community-based programs that bring together health, social and other supportive services in a protective, friendly and non-institutional setting. Each participant attends on a scheduled basis and receives the care detailed in an individual plan of service. Nursing, rehab, activites, counseling, exercise, socialization, nutritious meals and snacks, and arrangements for transportation to and from the program are all coordinated to help the participant maintain maximum independence. Centers provide respite to family caregivers as well as therapeutic care for older persons and persons with disabilities. Centers are open a minimum of 5 days a week, 9 hours a day. Some Centers also offer servfices in the evenings and on weekends.

  Any elderly or disabled person age 18 or older who is being abused, neglected or exploited is eligible.  APS investigates allegations of abuse, neglect and exploitation.  Investigates cases of self-neglect between 18-60 years of age.

  Service is free and confidential.  Computerized telephone system will call enrolled subscribers (older adults, disabled persons, shut-ins who live alone or anyone who needs to be checked on daily) at the same time each day to verify that they are okay.  If a subscriber fails to answer the phone for any reason, a contact person or local police will be sent to investigate.

     Eligibility:  Income guidelines and/or extenuating circumstances.

      Fuel assistance programs run from November-April.  Weatherization program provides home energy audits, heating system repairs and replacements and energy conservation improvements for qualified individuals.

  No Charge.  Provides rehabilitation, low vision orientation, mobility instruction for adults, volunteer services, peer counseling.  Braille tutoring.

  Services:  Commodity Supplemental Food program offering free monthly nutrition information and food—such as,

      canned fruits & vegetables, fruit juices, cereals and cheese.  To be eligible you must be 60+ years of age, living in

      Vermont and income-eligible.  Individual and/or household income will be used to determine eligibility.  Each

      household member may be eligible and will need to complete an application form.

  Income guidelines (2011):

    Elders: 1 person ($1,180/per month); 2 people ($1594/per month) – Each additional

     person in family, amount increases by $414/per person.

   Women/Children: 1 person ($1679/per month); 2 people ($2268/per month) – Each add’l

     person , amount increases by $589/per person.
