  Commodity Supplemental Food Program (CSFP) is a USDA nutrition program that offers FREE

  monthly nutrition information and nutritious foods--such as, canned fruits & veggies, fruit juices, cereals

  and cheese.

  Who is eligible: 60+ years of age, living in Vermont and income-eligible, a child under 6 years and not

  already in the WIC program. Individual and/or household income will be used to

  determine eligibility. Each household member may be eligible, and will need to

  complete an application form.

  Income guidelines (2008) are: Elders: 1 person ($1,127/per month); 2 people ($1517/per month)

  Women/Children: 1 person ($1603/per month); 2 people ($2158/per month)

 

Adult Day Services:

  AD Centers provide comprehensive programming specially tailored for older persons and persons with disabilities who need supervision and assistance during the day. They are non-residential, community-based programs that bring together health, social and other supportive services in a protective, friendly and non-institutional setting. Each participant attends on a scheduled basis and receives the care detailed in an individual plan of service. Nursing, rehab, activites, counseling, exercise, socialization, nutritious meals and snacks, and arrangements for transportation to and from the program are all coordinated to help the participant maintain maximum independence. Centers provide respite to family caregivers as well as therapeutic care for older persons and persons with disabilities. Centers are open a minimum of 5 days a week, 9 hours a day. Some Centers also offer servfices in the evenings and on weekends.
